Monthly reporting
Each participant has to fill in 2 forms monthly: an Activity report and a Timesheet. The forms may be filled in Czech or in English. Participant has to print out filled forms, signed them and let them signed by a supervisor. Forms has to be delivered to OPVVV Office (A.3.28a) by 10th day of the following month (either original or scanned original). In case of a stay abroad it is necessary to send the scans via e-mail to lenka.sterbova@uochb.cas.cz. The originals have to be sent or delivered personally once in half of a year to the OPVVV Office.
Activity report 
It is necessary to describe work in the activity report (the extent is from 1 paragraph to 1 A4 sheet) on monthly basis. The declaration of non-economic activity performance
remains the same during the stay: “The researchers declares that throughout the reference period he/she has performed solely non-economic activities.” Next part -
Interruption of work in the reference month – has to be filled in following manner: reason of absence – period of absence – number of hours when participant was
not working / performing activities related to the Mobility. The last part – Working time counted into the small-scale aid – is obligatory until participant
reach 320 hours of work. If so, participant has to fill in the box Monthly time allowance of the working time fund during the reference month that is working
time fund including public holidays. In the box Interruption of work in reference month participants fill in number of hours, when they were not at work
(public holidays are considered working days). In the box Total working time in the reference month participants state the total working time, i.e. working time
fund minus time of absence. The Cumulative working hours since the beginning of the Mobility is the sum of working hours in the referenced months since beginning
of the Mobility.
